Weight loss treatments can be grouped into

  2. Pharmacotherapy

Lifestyle modification through diet and exercise

  1. It is the preferred weight loss method for overweight people (BMI 23-27 kg.m2) or with class 1 obesity (BMI 27-32 kg/m2). Its goal is a moderate weight reduction of 5-10% of body weight within 6 months. The goal for weight loss should be 2 kg per month.

The diet should be limited to 1000-1200 kcal/day for women and 1200-1600 kcal/day for men. This would help in creating a caloric deficit of around 500-1000 kcal/day and help achieve the above weight loss goals without causing too much hunger.

The diet should be low in saturated fat, trans fat, cholesterol, added sugar and salt.

Foods to avoid include fried foods, baked goods, refined flour, sweets, red meat, egg yolk and whole milk and milk products.

Allowed foods include fried/baked foods, lean meats, egg whites, skim milk, green leafy vegetables, citrus fruits, and whole grain cereals and breads.

Exercise should be aerobic with moderate strengthening. Approximately 300 minutes of exercise per week is recommended for people who want to achieve a >5% reduction in their body weight. This exercise plan should be started gradually and initially done in 10-15 minute intervals to avoid fatigue. Daily activity levels should be increased and exercise such as brisk walking, jogging, cycling, dancing or swimming should be incorporated into the routine.

Behavioral changes include changing your environment to limit food intake and increase activity levels, keeping a food diary, self-motivation and support, and rewarding yourself with something other than food.

Weight loss drugs

Weight loss medications may be indicated for people who are overweight or obese but are unable to achieve their goals with diet and exercise. It is important to note that all medicines can have some side effects and should only be used for a short time as prescribed by a registered medical practitioner.

  1. Sibutramine – this drug results in weight loss of 5-10 pounds, but also causes serious side effects and has therefore been withdrawn from the market.

  2. Orlistat – Reduces the amount of fat that is absorbed from food and can be taken three times a day with food. It causes up to 8-10% weight loss after one year of regular treatment, but can cause side effects such as stomach cramps, flatulence and greasy stools in some people. It has also been linked to liver disease in several cases.

  3. Lorcraserin – Reduces appetite and the results are almost similar to orlistat, but it has other side effects such as nasopharyngitis, headache, respiratory tract infection, etc. It should not be given to patients with reduced kidney function and pregnant women.

  4. Phentermine

Other products such as Ephedra, Chromium, diuretics, herbal laxatives and hoodia have been associated with weight loss, but there is no reliable scientific evidence that these products actually help in long-term and effective weight reduction.

Surgery

It is only an effective treatment for long-term effective weight loss in morbidly obese individuals (BMI > 32 kg/m2 with associated comorbidities or BMI > 37 kg/m2 with or without associated diseases)
