Slider Foods Spell Weight Regain For Weight Loss Surgery Patients
For most people eating buffers is good. Popularized by the American food chain, White Castle, a slider (originally slyder) is a miniature grilled hamburger or cheeseburger on a steamed bun often served with onions and dill pickles and other condiments. They originally sold for a nickel a piece in the 1940s, making it affordable to add a side of fries for just pennies. By all accounts, this is a good “slider” type of food.
For the weight loss surgery patient, slide foods are the bane of good intentions and ignorance that often cause dumping syndrome, weight loss plateaus, and ultimately weight gain. Slider foods, for weight loss surgery patients, are soft simple processed carbohydrates with little or no nutritional value that slide right through the gastric sleeve without providing nutrition or satiety. The most innocent of foods are salt crackers, often eaten with hot tea or other beverages, to soothe the stomach in illness or while recovering from surgery.
Foods most commonly eaten include pretzels, crackers (salts, graham, Ritz, etc.) filled cracker snacks such as Ritz Bits, popcorn, cheese snacks (Cheetos) or cheese crackers, tortilla chips with salsa, chips, sugar free cookies, cakes and candy. You will notice that these foods are often salty and dry mouth so they must be swallowed with liquid to be palatable. This is how slider foods are made. Also, most of the time they lack nutritional value.
For weight loss surgery patients the digestive process is different than those who have not undergone gastric bypass surgery. When buffer foods are consumed, they enter the stomach pouch and exit directly into the jejunum, where the sludge of simple carbohydrates is quickly absorbed and stored by the body. There is as little thermic effect in digesting simple carbohydrates as there is in digesting protein, so little metabolic energy is expended. In most cases, patients in the weight loss phase eating buffered foods will experience a weight loss plateau and possibly regression of weight gain. And unfortunately, they will start to believe that their surgical stomach pouch is not working properly because they never feel full or restricted like they do when they eat protein.
The very nature of the surgical gastric pouch is to cause feelings of tightness or restriction when one has eaten enough food. However, when soft simple carbohydrates are eaten, this tightness or restriction does not occur and one can continue to eat countless copious amounts of non-nutritive foods without ever feeling uncomfortable.
Many patients turn to slider meals for this very reason. They don’t like the discomfort that results when the pouch is full from eating a measured serving of lean animal or dairy protein without liquid. However, this very limitation is the desired effect of the operation. Discomfort is meant to signal the cessation of eating. Remembering the “Protein First” rule is crucial for weight management with bariatric surgery.
Gastric bypass, lap-band, and gastric sleeve patients are instructed to follow a high-protein diet to facilitate healing and promote weight loss. Bariatric centers advise what are popularly known to weight loss surgery patients as the “Four Rules,” the most important of which is “Protein First.” This means that of all the nutrients (proteins, carbohydrates, fat and alcohol) the patient must eat protein first.
Protein is not always the most comfortable food choice for weight loss surgery patients who feel restricted after eating very little food. However, for the surgical instrument to function properly a diet high in protein and low in simple carbohydrates buffer foods must be followed. The high protein diet should be followed even after reaching a healthy weight in order to maintain a healthy weight and prevent weight regain.
