10 Days With a Numb Lip After a Visit to the Dentist

I am entering my second year of engineering. This transition year will take me from a broad-based general education to a more highly focused mechanical engineering program through advanced studies in mathematics, science, and systems. By all measures, I was ahead of my peers and my grades were above average. My freshman year is over, and I’m actually pacing myself to the complexity of projects that will eventually see my skills put into practice. At this stage of my studies, I was like a sponge in the ocean, soaking up all the classes I took. Pulled and pushed by every movement of the tides around me. My internal GPS did not calculate a clear course; however, there was a steady wind that seemed to be pushing me in a definite direction. Or so it seems to me.

It’s hard to hide who I am; I’m from a small town in the Midwest and went to a private high school with my siblings, where we enjoyed a comfortable life with professional parents. My mother is a restorative hygienist and my father, Dr. Watson, owns a well-known dental practice. I’m off campus now, and I’m lucky enough to share a three-story loft-style apartment with Steven, also an engineering student a year ahead of me. Our apartments are no larger than 500 square feet. The living space, but comfortable and spotless, has a clear view across the town, where the familiar golden arches can be seen, where many college students take notes while enjoying the special burger of the day. Steven is no exception. He likes to eat at McDonald’s. He’s obsessed with their Big Macs and eats at least four or five a week. Our apartment consists of a lounge area, kitchen, bathroom and a bedroom with 2 single beds. I’m starting to feel like I have more control over my life and can come and go as I please. My roommate is friendly, but somewhat quiet and deep in his mind, at least it seems to me.

We have totally different schedules and don’t get much time together other than the occasional small talk over coffee. We’re not much of a drinker but enjoyed this odd beer. He works part-time at a small computer repair shop and is usually home by midnight. Northeast’s rhythm and anonymity are captivating, and every day I can feel a maturity seeping through my every move. I feel light and happy with myself, especially when I hear from my family. Although never overtly imposed on me, the atmosphere at home in high school subtly foreshadowed the expectation of continuing my undergraduate studies in the East. I was the eldest of my siblings and I can reflect now that both my parents had me as their flagship. “If Jim leads the way, others will follow”. I’m lucky because I don’t have to work during the term away from home. My mom saw it. There are no excuses for failing grades. I lack nothing. But I am by no means pretentious. In fact, I am often seen and not heard. In the back of my mind, I know I’m welcome, even though I’m shy. I’m not “Brad Pitt,” but I’m confident, at least in my opinion.

In the second semester of my sophomore year, I started worrying about my roommate. He suffers from bruxism, commonly known as bruxism, which is usually accompanied by clenching of the teeth. It is an oral parafunctional activity that is common in many people. Bruxism (Wiki) is caused by activation of the reflex chewing activity; it is not a learned habit. Over the years, I have learned many dental terms from my parents by osmosis. I’m not new to this situation (grinding); however, this situation keeps me up at night. During our weekly “how are you” conversation, I talked to my dad and he suggested that Steven should see a local dentist because his teeth might be damaged. It wasn’t hard to share my concerns with Steven, since his girlfriend also thought he should go to the dentist.

We (Steven, his girlfriend Veronica and I) are getting closer now. Steven is a little more open and can be funny at times. I saw another side of him. We started hanging out more often. Steven appreciated my dad’s professional advice and did visit the dentist associated with our engineering school. Diagnosed. There were some small initial lesions that had begun to eat away at the enamel of his molars. The dentist was able to fit a guard over Steven’s mouth so he could sleep comfortably at night. A follow-up appointment was scheduled for a few weeks later to ensure the protective gear was comfortable to wear. However, the dentist noticed an old amalgam filling on his LR bicuspid which was overhanging and was a flosser. The dentist suggested that Steven should be replaced with a more modern white filling. Amalgam is mostly obsolete now. The trend is for compound fillings in natural colours. This is good news for Steven, because amalgam can be cumbersome and unsightly for young people. The date to replace the filler is set and everything will be fine, at least it seems to me.

Steven’s 9:00am dental appointment would have given him plenty of time to get back to class, and by noon, the anesthesia would wear off, giving him plenty of time to enjoy his Big Mac. However things didn’t go as planned. Steven’s jaw was still numb, and although he tried to ignore the strange feeling that persisted, it was clear that he had indeed lost interest in his favorite Big Mac and chips. On the third day, Steven called his dentist and jokingly explained that he had lost his appetite for any food, especially the Big Mac craving. The dentist assured him that some people had this reaction to the local anesthetic, but the numbness and tingling would soon pass and return to normal, full sensitivity.

Steven took his word for it with skepticism, but after the fifth day things weren’t going any better and Steven panicked and wanted to hear someone else’s opinion. Without knowing the whole picture, my dad had the good fortune to only be able to “hear” and warn me that this could be a paresthesia condition, a potentially serious pitfall that dentists hope will never be in their practice In such cases. For obvious reasons, my dad didn’t want to get involved. I am now in a dilemma. After all, Steven was my trusted friend and roommate.

Unexplained paresthesias:

Steven is now withdrawn and depressed. He lost weight and his papers sucked. Veronica accompanied him to a counselor, who decided to send him to a neurologist. Paresthesia is actually a diagnosis. The situation was explained to Steven and the prognosis was good as he had a simple and uncomplicated anesthetic infiltration and was not exposed to a complete nerve block. There are still unresolved issues. Why does this happen?

Around this time, the FDA approved a new local anesthetic for dental use in the United States. Unlike most other anesthetic solutions that are 2% concentrated, this new local anesthetic is 4% concentrated. In other words, theoretically and pharmacologically, all things being equal, or as scientists say, in vitro, one could use half the volume of a 4% solution to achieve the capacity of a 2% solution. For some unknown reason, dentists are used to using volume (cartridge) instead of mg/ml. (actual mg given).

As a result, Steven woke up on the tenth day to find that his paresthesias had miraculously reversed, and his tongue and the soft tissues around his lips had regained full sensitivity. Not to mention where he went after he found out there were no more pins. All therapeutic compounds are accompanied by an index. The lesson to be learned is that the product monograph contained in any drug emphasizes the implications of the minimum effective dose.
